What insurance do you accept?
Pure is a performance physical therapy provider that prioritizes your health outcomes over insurance allowable reimbursements. This approach enables us to provide with multi-modal therapy techniques and customized treatment plans that can help accelerate your recovery.

While we aren’t currently billing insurance for treatment, many of our clients have successfully submitted for reimbursement from their providers
What are your rates?
Our standard rates are $80 for 1/2 hour session or $150 for one hour.
All new patients or clients returning with new condition will require a 1 hour session for full condition assessment and performance plan creation.
Can I be reimbursed by my insurance?
We do provide you with a "super bill" for insurance reimbursement and the majority of our clients are successfully reimbursed. However, we do not guarantee reimbursement from your insurance company. To better understand your out-of-network benefits please contact your health insurance team. 
How many visits do I need?
This can vary greatly depending on several factors including the location and severity of your injury. At your initial visit we will do a full assessment and develop your personalized performance therapy plan. This will provide you with more information around the length of your treatment, modalities likely to be used, as well as at-home activities that can aide in your recovery.
How long will it take to recover?
This varies depending on the location and severity of your injury, the frequency and types of treatments used, your individual health factors, as well as participation in your performance treatment plan's at-home activities.

Zack Fedorka

DPT, FAAOMPT, CSCS, Cert DN

Zack Fedorka brings over a decade of clinical experience in physical therapy, specializing in personalized, one-on-one care designed to optimize performance and recovery. He earned his Doctorate of Physical Therapy (DPT) from the University of Toledo in 2014 and pursued advanced education through an ABPTRFE-accredited residency at the Manual Therapy Institute, completing it in 2017. In 2018, Zack became a Fellow of the American Academy of Orthopaedic Manual Physical Therapists (FAAOMPT), solidifying his expertise in advanced manual therapy techniques.

In addition to his physical therapy credentials, Zack is a Certified Strength and Conditioning Specialist (CSCS) and is certified in Personalized Blood Flow Restriction (BFR) Therapy through Owens Recovery Science. His commitment to staying at the forefront of innovative care led him to gain certification in Dry Needling through the American Academy of Manipulative Therapy. He also had the opportunity to serve as part of the athletic training staff for the Tennessee Titans during the 2019-2020 season, providing him with unique insights into high-performance sports therapy.

Too often, PT is commercialized, with group sessions and divided attention. Zack is passionate about breaking away from that model and providing high-quality, focused care to each patient. His approach emphasizes empathy, accurate diagnosis, and individualized treatment, allowing him to fully understand his patients’ unique conditions and deliver optimal results. He believes in dedicating undivided attention to every client to enhance healing and performance outcomes.
